Comparing a Catheter versus Wire-Guided Technique for Accessing a Person's Bile Duct

The bile duct is accessed through an opening called the ampulla during a procedure called an endoscopic retrograde cholangiopancreatography (ERCP). Two techniques used for accessing the bile duct include a wire-first technique and a catheter-first technique. The wire first technique is usually more commonly used in the United States. This study will compare the use of a catheter to first access the bile duct, rather than a wire, to see if it reduces the risk of complications from the ERCP procedure. People in the study will be randomly assigned to receive the wire-guided technique or the catheter-guided technique to gain access to the bile duct during the ERCP procedure. Being in the study from the time of the procedure lasts about 4 weeks. Participants will be contacted by phone after the procedure.

Inclusion Criteria
- Ages 18 years and older
- ERCP with no prior sphincterotomy surgery to access the bile duct for any sign except distal malignant (bowel-related cancer) obstruction from pancreatic malignancy (cancer)
Exclusion Criteria
- Altered surgical anatomy that interferes with inserting a catheter or wire into the bile duct
- Had ERCP to treat or access the pancreatic duct
- Had sphincterotomy surgery already been performed
- Allergy to iodinated contrast (medical dye that contains iodine to improve seeing blood vessels and organs in the body)
- Pancreatic malignancy (cancer)
